چکیده

We report two patients with massive intratumoral hemorrhage in pineal tumors. A 23-year-old male with pineocytoma, whose (MRI, CT) image demonstrated obstructive hydrocephalus, was subjected to a right ventriculoperitoneal shunt. His consciousness deteriorated, and he experienced massive intratumoral hemorrhage and later died. Another case of pineal tumor was a 47-year-old male who underwent emergency external ventricular drainage for associated obstructive hydrocephalus and deteriorating consciousness. He had a sudden deterioration following the procedure and imaging revealed massive intratumoral hemorrhage. He remained vegetative at discharge and died 2 weeks later. Although Intratumoral hemorrhage in pineocytoma after the placement of a ventriculoperitoneal shunt or external ventricular drainage is rare, it is an important cause of morbidity and mortality and should be kept in mind when cerebrospinal fluid (CSF) diversion was conducted before a craniotomy.

A Prospective Study To Evaluate The Role Of Perioperatve Cerebrospinal Fluid Diversion By Lumbar Drain In Transnasal Transsphenoidal Tumor Surgeries

Background &Aim: The endoscopic endonasal transsphenoidal approach has been widely employed as a treatment for pituitary adenomas and other parasellar tumors. However, the major concerns related to endoscopic endonasal transsphenoidal surgery include the risks of CSF leak post-operatively leading to morbidity and at times mortality due to severe meningitis. Several studies in the past have foun...

Diagnosing early upward cerebellar herniation by computed tomography: A diagnostic boom, a savior

Preoperative diversion of cerebrospinal fluid (CSF) is a common procedure for posterior fossa lesions. Although rare in incidence, upward cerebellar herniation may occur after such procedures, and is associated with a high mortality. Most of these patients present with sudden apnea and neurological deterioration with imaging showing dense intratumoral hemorrhage.[5] We bring to light a similar ...

Dural Arteriovenous Fistula Following Translabyrinthine Resection of Cerebellopontine Angle Tumors: Report of Two Cases

We describe two cases of dural arteriovenous fistula (DAVF) developing in a delayed fashion after translabyrinthine resection of cerebellopontine angle tumors. Two patients in an academic tertiary referral center, a 46-year-old woman and a 67-year-old man, underwent translabyrinthine resection of a 2-cm left vestibular schwannoma and a 4-cm left petrous meningioma, respectively. Both patients s...

Pineal Region Tumors

The pineal region can be affected by different types of lesions, ranging from benign masses (e.g. pineal cysts) to highly malignant tumors. These neoplasms are typical of the pediatric age, while they are rarer in adults. Due to the extreme heterogeneity of the cell types of the pineal gland and its surrounding structures, these tumors include several entities, namely germ cell tumors (GCTs), w...

METASTATIC SPINAL INTRAMEDULLARY MEDULLOBLASTOMA Case Report

Many intracranial malign tumors including glioblastomas, malign astrocytomas. medulloblastomas. some of the pineal region tumors and ependymomas have a high potential for metastasis (3.4,11). These tumors usually spread via the cerebrospinal fluid pathways and locate on the surface of the spinal cord. Pure intramedullary location of a metastatic mass from medulloblastoma is a very rare patholog...
